You can also Log In to the Member Portal to find doctors that accept your plan.

Alert: Effective, November 1, 2023 PCP to specialist referrals will resume for the following plans:

• AvMed’s Engage and Entrust Individual and Family plans
• AvMed’s Medicare Advantage Plans Choice and Circle plans

With the End of the COVID-19 Public Health Emergency, effective August 1, 2023, AvMed will be reinstating its PCP to Specialist referral requirement for Medicare Advantage Plans Choice and Circle.

To increase ease of the referral process, we've also increased the amount of time a referral is valid for from 90 days to 180 days. Your PCP can begin obtaining referrals now for services you'll have on or after August 1, 2023.

You can order a new card by viewing your digital ID card or printing a temporary ID card in the Member Portal. After you order a new card, it should arrive within 7-10 business days.

1. Visit the login page
2. Find the “Account Login” box
3. Click the “Forgot User ID or Password?” link. The “Register for an Online Account” page will display.
4. Choose the first option in the middle of the page, “Members,” and then choose “Password Reset Page.”

Quantity limits are set in accordance with the FDA, which means different prescriptions may have limitations.

For chronic conditions like asthma, cardiovascular disease and diabetes, you have the opportunity to obtain a 90-day supply with prior authorization. Check prior authorization and get more information.
